Domestic Homicide Review

When there are deaths due to domestic violence the law requires professionals working in the area the deaths occurred to convene a meeting to advise if the person/s killed (there may be more than one individual on occasions e.g. mother and child/ren) was known to their organisation.

The Home Office has published guidance on when a domestic homicide review (DHR) should happen and how to do it.

(Domestic homicide reviews are not inquiries into how the victim died or into who is responsible. The purpose of a domestic homicide review (DHR) is to understand where there are lessons to be learned and to make recommendations to prevent future homicides.

An individual who is independent of all local agencies is requested to chair a series of panel meetings and collate details of agencies involvement.  The independent chair will then write a report which will make recommendations in areas that local agencies need to improve.

Domestic Homicide Review Homicide Report - Bromley Susan

This report is the domestic homicide review for ‘Susan’ (this is not her real name). This was the first domestic homicide review in the London Borough of Bromley following the implementation of the statutory guidelines in April 2011.

The DHR was commissioned to be conducted by the Safer Bromley Partnership to an agency called Standing Together against Domestic Violence (STADV) who works to promote a coordinated, multi-agency community response to domestic violence.  Anthony Wills, an associate of STADV completed the DHR.

This followed the police informing the partnership of ‘Susan’s’ death.  They were required to make a decision on whether a domestic homicide review should take place, and if so, appoint an independent chair and report writer. Panel members included the local authority and the police.  A full list of all panel members can be found listed in the report.

All of the information shared for the review is confidential until it has been approved and published.  An action plan has been developed locally to ensure implementation of recommendations made in the DHR.

Family members, friends and colleagues of the victim are important to the DHR process and the independent chair aims to make contact with them to help inform the review and build a complete picture of the victim.

This report was sent to the Home Office for approval in July 2015 and the Home Office Quality Assurance Panel advised that it had been approved for publication in October 2015.

Domestic Homicide Review Homicide Report - Bromley Mary

This report is the domestic homicide review for ‘”Mary” (this is not her real name). This was the second domestic homicide review in the London Borough of Bromley following the implementation of the statutory guidelines in April 2011.

The DHR was commissioned to be conducted by the Safer Bromley Partnership to an agency called Standing Together against Domestic Violence (STADV) who works to promote a coordinated, multi-agency community response to domestic violence.  

This followed the police informing the partnership of “Mary’s” death.  The Chair of the Safer Bromley Partnership was required to make a decision on whether a domestic homicide review should take place, and if so, appoint an independent chair and report writer. Panel members included the local authority and the police.  

All of the information shared for the review is confidential until it has been approved and published.  An action plan has been developed locally to ensure implementation of recommendations made in the DHR.

Family members, friends and colleagues of the victim are important to the DHR process and the independent chair aims to make contact with them to help inform the review and build a complete picture of the victim. Following discussions between the Chair and the family, and with the agreement of the Home Office, it was agreed the publication of the whole report would not be suitable in this case and the recommendations and action plan are available for review.

Domestic Homicide Review Report - Bromley Alice

This report is the domestic homicide review for ‘”Alice” (this is not her real name). This was the third domestic homicide review in the London Borough of Bromley following the implementation of the statutory guidelines in April 2011.

The DHR was commissioned to be conducted by Sancus Solutions following a referral by police informing the Safer Bromley Partnership of “Alice’s” death.  The Chair of the Safer Bromley Partnership was required to make a decision on whether a domestic homicide review should take place, and if so, appoint an independent chair and report writer. Panel members included the local authority, probation and the police.  

All of the information shared for the review is confidential until it has been approved and published.  An action plan has been developed locally to ensure implementation of recommendations made in the DHR.

Family members, friends and colleagues of the victim are important to the DHR process and the independent chair aims to make contact with them to help inform the review and build a complete picture of the victim.
