Home from hospital services provide extra support to people who have been discharged home from hospital following surgery, ill health, or an accident. They are short term aftercare services and can offer flexible short-term, practical help to assist with most day-to-day tasks, such as emotional support, food preparation, shopping, laundry, light domestic duties, and help with claiming benefits and form filling.

Different health, social care, voluntary and commercial organisation can provide support for a few days to a couple of weeks, to ensure you regain confidence and re-adjust to living independently in your own home.

Older people support services

Bromley Well provide practical, emotional, and planning support that promotes independence and help to improve your wellbeing, from friendship groups to our handyperson service. The service also helps older people who are vulnerable and/or mildly frail.

Support provided includes information advice and guidance, adult carers, community friendships for people aged 65 and over, Dementia Respite at home service, a handyperson service and a 6-week aftercare service. Please note some services are only available via professional referral. South East London(SEL) Mind

South East London Mind provides a number of activities, from a range of community venues, including:

  • A programme of courses and support groups, including support to better manage your mental health, develop new skills, enhance creativity, and take up leisure opportunities.
  • Community bridge building, which is support to make links and begin to access a range of opportunities in the wider community, according to your interests.
  • Informal peer support, which provides opportunities to meet with others in a friendly, safe and welcoming environment for social and peer support.

Bromley Dementia Support Hub and Mind Care Dementia Support

The Bromley Dementia Support Service provided by SEL Mind, offers a range of services including support centres, domiciliary care and respite care at home.

Bromley Dementia Support Hub offers the following:

  • Information, advice, and assisted referrals (triage) - An information and advice phone line where users/carers are provided with information.
  • Dementia advisors - who provide person centred help and support to service users and their carers.
  • Young onset dementia support - A variable service targeted at those with a dementia diagnosis aged 64 and under.
  • Befriending - A dementia volunteer befriending service, which works with people with dementia and their carers.
  • Training and workforce development - providing specialist dementia training for carers and professionals.
  • Memory Lane café and peer support - Dementia cafes run by AGE UK which are Day Centres specifically for people with dementia and their carers.

Mind Care Dementia Support

Provides individually tailored support in the home, giving families and friends a break from caring for someone with dementia.
