This form is for use by GP's, hospitals or health professionals only. It can be used to request, amend or cancel a regular clinical waste collection.
Please note that items such as incontinence pads and soiled bandages and dressings can be double-bagged and deposed of in a patients normal domestic waste collection. Clinical waste referrals for regular collections should only be where there is risk of infection from the items or sharp instruments.
