The Bromley Community Learning Disability Team (CLDT) provides specialist community health services to local people over the age of 18 with learning disabilities. The team consists of psychologists, community nurses, occupational therapists, physiotherapists, speech and language therapists and psychiatrists.
Holistic care is provided to people with a learning disability through a range of interventions including:
- Complex physical health (CPH) care
- Mental health care
- Epilepsy care
- Positive behavioural support (PBS)
- Supporting transition from child to adult health services.
How to be referred
All referrals are subject to the team’s eligibility criteria. The eligibility criteria are as follows:
- Aged 18 or over – and
- Learning disability diagnosis confirmed – and
- Registered with a local GP – i.e. within Bromley – and
- Presenting with one or more additional health needs
Complexity is defined as:
- The impact of the learning disability on the additional health needs is such that management of the condition goes beyond the capacity of mainstream NHS services
- Direct and frequent contact with adult learning disability health services is required to ensure the stability of the condition and/or improve health outcomes.
In the final year of school, if a young person is eligible for adult learning disability services, the transition nurse will complete a holistic assessment of their needs and will make all relevant health referrals into adult services to ensure continuity of care.
The transition nurse will talk to you about future plans after school and help with this transition where possible. If you have an Education Health and Care plan they will contribute to the health part of the EHCP to ensure that this contains up to date health information. The transition nurse will also provide you with a hospital passport and a black book which is a place where you can keep all health related information.
