Who are we?
The Preparing for Adult (PfA) team are a social care team for young people with disabilities in Bromley within the 0-25 service. If you have a diagnosed disability (learning, physical, sensory impairment or autism), we may be able to support you in preparing for your adult life.
How we can help
Before you become an adult, the PfA team and members of the 0-25 service can offer advice and guidance to you, your family and other professionals (such as EHCP co-ordinators, Teachers and Social Workers). Where we know about you, and you are on the pathway, the 0-25 service aim to take part in your EHC plan school reviews, from Year 9.
As you turn 17, we may be able to offer a Care Act Assessment. This is where a PfA worker will meet with you to find out things like:
- The things and people that are important to you in your life - what you are good at, what you enjoy, how you can be involved in your community (for example meeting friends, enjoying hobbies, having a job).
- Your practical, everyday skills – for example managing money, travelling, staying safe and washing and dressing and what help is needed for you to achieve these skills.
At the end of the assessment, we will have an idea of what you want to do as an adult along with any support that you may need and any potential for future independence.
If you are viewed as being eligible, we will complete a support plan to meet these needs.
If you are not eligible, we will signpost you to other services that might be able to help, such as Bromley Well.
How to let us know about you
If you have an EHC plan and a diagnosis of learning disability, autism, a physical disability or a hearing or sight impairment, we can accept referrals from:
- Directly from yourself/family member
- Children's Social Worker
- School/EHCP co-ordinator
They should alert you to us when you start Year 9.
